This booklet is published by the Northern Berkshire eHealth Steering Committee. This committee represents many health care providers who are working together on a pilot project to develop a communitywide electronic health network. The goal of this project is to improve health care quality, cost, and safety. 

Notification of change to Electronic Health Records

This documents the important information on electronic health records and the changes that will be made in doctor's offices in the Northern Berkshire Community.

Levels of Consent  

In order to satisfy applicable Massachusetts regulatory requirements surrounding disclosure of certain classes of clinical information, this policy outlines the procedures, documents and technology solutions that will be used in this regard. 

Privacy and Security Monitoring and Sanctions

Purpose is to set forth the Northern Berkshire eHealth Summary policy and procedures for the monitoring of patient privacy and data security. This policy and procedure will also address the issue of what sanctions will occur when a violation of patient privacy or data security has been identified.

Mitigation and Notification Policy for Privacy and Security Violations

Purpose is to set forth the Northern Berkshire eHealth Collaborative’s policy and procedures regarding the mitigation and notification of privacy and security violations of the eHealth Summary (eHS).

Consent Form

Consent form that grants permission to create an eHealth summary and share medical information

Security Login Account Form

Form employees must sign related to the use and operation of the eHealth Record (EHR) and the eHealth Summary.  These polices, as well as others are designed to protect the rights of patients, as well as the practice and its employees.
